In-Patient Guide

OVERVIEW:

Thank you for choosing Menezes Polyclinic. We request your co-operation to enable the management to provide you with the best health care. The following information will assist you to understand the system of admission and hospitalization, which are essential for your convenience & speedy recovery.

ADMISSION:

You will require ‘Admit Instructions Slip’ from the Hospital Doctor/Doctor attached with the Hospital to register for admission. This slip will be given to you by the Doctor in the Outpatient Consultation room or in the Casualty Department of the Hospital or at the Doctor’s own Clinic. This slip is to be produced at the Admission Counter where formalities will be completed after payment of an advance deposit. Patients are requested to insist on a receipt for all payments made. A refundable Rs 100/- Patients Attender Pass may be obtained for patients opting for private and deluxe rooms only. This is valid for the entire stay of the patient.

ROOMS:

Various categories of rooms from General Ward, Semi Private, Private & Deluxe Rooms are available. All efforts will be made to provide the rooms of your preference but in case of non-availability, an alternate room will be allotted until such time the room of your choice is available.

Hospitalization charges depend on the type of Ward/Room occupied by you. It is important to note that the charges for procedures / surgeries vary based on the type of accommodation selected. You are requested to ascertain the same before admission.

When a patient is admitted to the ICU or is shifted to the ICU from the Ward/Room, the attendant/relative should vacate the Ward/Room. However the patient’s attendant/relative in Private Rooms may retain the room with prior permission of the Management, in case the same is not required by another patient, on payment of applicable Room tariff besides the ICU Charges.

TARIFF:

Hospital stay is charged on a ‘per day basis’ and if the stay is extended beyond 6 hours, patient will be charged for the full day. In case the patient occupies the room for less than 6 hours, half day charges will be levied.

Ward/Room tariffs include bed charges, nursing charges & patient diet charges. Patients are requested to check and verify the financial implications before requesting for a particular type of Ward/Room. Detailed estimate for hospitalization expenses will be provided by the billing department.

In case a patient shifts from one category of Ward/Rooms to another, all charges for the entire hospitalization will be levied at the rates as applicable for the higher category of Ward/Room occupied.

DEPOSIT:

On admission, an initial deposit will be collected at the Inpatient Counter. This amount will depend on the category of Ward/Room and the treatment/surgical procedures planned. Reminders for further deposit will be sent from time to time in the manner the credit is utilized. Payment will be accepted only by Cash. No Cheques are accepted.

Please understand that it is mandatory to make an appropriate advance deposit to avoid any problems at a later stage of your hospitalization. Please note that the Admission/Casualty staff is instructed by the management to collect deposit before hospitalization.

All Government of Goa Mediclaim patient will be required to produce the formal authorization letter in Form ‘E’ addressed to the Menezes Polyclinic and issued by the Director of Health Services, Govt. of Goa, permitting them to avail the facilities at the Menezes Polyclinic. This authorization letter should be produced on admission, otherwise the patient will have to pay the full amount to the Hospital and then claim medical reimbursement from the concerned authorities.

It should be noted that the authorization letter has a sanction limit which the patient is entitled. However, in an unlikely event should the amount sanctioned by the Government be insufficient for the complete treatment, the patient shall be informed accordingly and he/she will have to make the necessary arrangements for payment of this additional amount which will have to be paid prior to discharge. It is advisable that the patient, especially in Cardiac cases, gets prior information on the probable amount to enable him/her to make the necessary arrangements in such an unlikely event. Except for dire life/limb saving emergencies, payment of advance deposit is mandatory and non-payment may lead to refusal of admission.

CASHLESS INSURANCE

In case, a patient wants to avail the benefit of cashless Insurance, he/she have to produce the required documents such as policy copy, cashless ID card etc. to the admission counter. Only on receipt of the cashless authorization letter from the insurance company/TPA, the patient shall be given the benefit of cashless insurance subject to terms and conditions as applicable. The hospital will extend the benefit of cashless insurance only with insurance companies or Third Party Administrator(TPA) empanelled with the hospital. The patient will have to pay the balance amount over and above the limits sanctioned for cashless benefit

Billing Department / Floor Co-coordinator will keep the patient informed of the approximate bill amount and the quantum of advance that would be required to be paid periodically. It should be noted that this again is only a gross approximation. Final bill at the time of discharge will be made after inputs are available from all the departments.

FINANCIAL ESTIMATES FOR HOSPITALISATION:

Financial estimates will be valid only when the Billing Dept. issues the same in writing. Estimates issued by any other functionary will not be honored by the Hospital. It should be noted that charges for the procedures will depend on the category of room occupied by the patient.

Whatever estimates are given to the patient are only gross approximation. An accurate estimate cannot be given because of the large number of the variables. For example, an estimate for an operation is given for one specific procedure and for average time taken for surgery. If at the time of surgery an additional procedure becomes necessary, the surgeon’s charges and instrumentation charges would automatically increase. If for any reason the time taken for surgery exceeds the average time, the OT charges would escalate. Similarly additional investigations like blood test, X-rays, scans, etc. may become necessary and this could increase the billing. It is therefore an earnest request that patients should be aware of possible variations in the estimate given at the time of admission.

Estimated cost does not include cost of medicines / consumables. Both are highly variable.
Additional procedures, extra stay and involvement of more than one specialist / super- specialist in the treatment, will naturally involve additional cost.

CHANGE OF ROOMS:

If a patient moves to a higher category room, the charges for procedures / Consultant’s fees will be as per the higher category of room and patients are advised to acquaint themselves with these charges at the time of admission or before requesting for a change in the room.
However, if a patient moves to a lower category room after a surgical procedure, the charges for procedures / consultants fees shall be as per the higher category room occupied before the procedure.

ATTENDANTS:

Patients in Private Rooms are permitted to have one attendant with a pass. The pass is provided on admission. “SECURITY” will check all entry points and at random to control unauthorized persons from loitering in the Hospital. Please co-operate with them as it is in your interest and they are only doing their duty.

The Patient’s attendant / relative will have to produce their pass when demanded. The attendant / relative will be provided with a couch for his/her use. In Private rooms, pillows and sheets will be provided. No linen from home will be permitted.

No male attendant will be permitted to stay overnight in the female ward. Patients and their relatives are requested not to argue or indulge in any type of altercation with the security staff on this account.
Patient’s attendants will not be allowed to stay in wards from 7pm to 8am.

The Hospital has special A/C. Lounge with bath / Toilet facilities for the use of attendants / relatives of patients admitted in the ICU. Rs. 150/- per bed, per night will be charged for the use of the Lounge, a receipt for which will be issued by the Hospital for that particular night only.
Besides lounge facilities the hospital has dormitory facility for use of attendants/relatives of patients. Rs 50/- per bed, per night will be charged for use of the dormitory facility. Patient’s attendants/relatives may use the cafeteria located in the hospital premises. The same will be open for lunch from 12.30pm to 2.30pm and for dinner from 7.30pm to 9.00pm.

DIET:

Diet is provided to the patients as per the Doctor’s advice. Outside food is not permitted in the Hospital.
Consumption of alcohol and tobacco is strictly prohibited in the Hospital. Visitors are also not permitted to bring flowers in the Hospital.

FACILITES FOR PATIENTS:

Nurses Call System in the Rooms: Nurses Call System is provided to all patients in the Rooms. You can either press the ‘call bell’ button for the nurse or use the phone by your bed side.

Telephones: House telephones are provided in Private rooms and in all Ward Lobbies. Local calls can be made by requesting the Telephone Operator (dial 9) at a cost of Rs. 5/- per call of 3 minutes.

Television: T.V is provided in all Private / Air-conditioned rooms. Patients are requested to use the T.V. in such a manner that it does not disturb other fellow patients.

Private Nurses: The hospital is staffed with fully trained and qualified nurses and it is our endeavor to give maximum attention to our patients.

Lifts: Lifts are provided primarily for the use of patients and Hospital’s Consultants. Patient’s attendants and visitors are requested to use the staircases.

ATM:

ATM services are available on the premises.

VISITORS:

General: Visitors are permitted to visit their patients during the approved visiting hours. During visiting hours only two visitors at a time are allowed to be with the patient.
Visiting hours are from 4.00 p.m. to 7.00 p.m. every day and on Sundays and holidays from 10.00 a.m. to 12.00 noon & 4.00 p.m. to 7.00 p.m.
Visitors are requested to confine to the patient’s room and not crowd in the corridors. By 7pm all visitors must necessarily leave the hospital premises. The security will ensure that the visitors leave by 7 pm.

I.C.U.: For I.C.U. patient’s visitors timings are from 7.00a.m. to 8.00a.m. . and 4.00 to 6.00 p.m. Only close blood relatives will be allowed to visit the patient in the I.C.U. and that too one visitor at a time. Visitors are requested not to pressurize the security personnel to gain entry in the I.C.U. This is in the interest of your relative and other patients in the I.C.U.

VALUABLES AND PERSONAL BELONGINGS:

Please avoid bringing valuables like jewels, watches, mobile phones, purses, etc. to the Hospital. Please be assured that while all possible help will be rendered to patients and their attendants to retrieve stolen belongings, the hospital will not be responsible for any financial compensation for loss of personal belongings, especially mobile phones.

DISCHARGE FROM HOSPITAL:

A patient can be discharged from the Hospital on the advice of the Consultant. After the discharge intimation is sent by the ward sister to the Billing Section, it would take approximately 1-2 hours to secure clearance from all Departments such as Pharmacy, Housekeeping, Operation Theatre, Laboratory etc. The Billing Assistant will inform the patient when the bill is ready and the patient is requested to bear with this inconvenience which is inevitable.

Before departure, the ward sister will hand over the prescription and explain to the patient about the treatment to be continued.

PAYMENT OF BILLS:

All payments to be made strictly in cash or through approved credit card only. Cheques are not accepted. CGHS, Goa State Government’s Mediclaim, Company referred and other Insurance patients will have to sign all required  documents prior to  being discharged. On payment of the bill/clearance of credit bill formalities, a discharge  slip duly stamped and signed by the billing executive  shall be given to the patient who in turn will have to produced the slip to the ward nurse and thereafter submitted to the security personnel before leaving the hospital premises.
